Activities Release

The undersigned parent or guardian hereby releases and indemnifies Villa Madonna Academy from any and all liability for claims arising out of injuries related to participation by the student in activities conducted or sponsored by Villa Madonna Academy. 

I understand the personal safety of the student is of first imporance to the school.  In the event of needed professional medical care, I give my permission for a representative of the school to transport my child to the nearest medical facility to render treatment. 

I will complete the 2025-2026 Emergency Card and return it to the school so it can remain on file.  (These will be distributed at packet pickup). 

We have medical insurance to cover accidental injuries.*
