Villa Madonna Academy Student Publicity Agreement Form

I give permission for Villa Madonna Academy to include my child’s likeness, name, voice, and work samples in school-produced publications including newspapers, newsletters, booklets, video productions, pod-casts, and the school yearbook. (Your child’s photo cannot appear in the school yearbook unless you grant this permission.)

I give permission for Villa Madonna Academy to post my child’s likeness, name, voice, and work samples on the school’s website.  Examples include student videos, Villa Week, and web pages showing student achievements or school activities.

I give permission for Villa Madonna Academy to include my child’s likeness, name, voice, and work samples in press releases, school advertisements sent to the public media and public media websites such as NKY.com and KyPost.com, and the school social media pages. Press releases generally pertain to students winning awards, academic or sports contests, appearing in school productions, or participating in extraordinary extracurricular activities.

I give permission for Villa Madonna Academy to publish my name and likeness if photographed or videoed in conjunction with a school event.  Examples include parents helping at school-sponsored events and parents with their children at school productions.
